What do you need to register?
1. Appropriate Fee
2. Proof of Birth for first time MYSA players (birth certificates, Passport, Alien Registration Card; NO hospital records)
3. Passport sized picture of player's face 1" x 1 1/2" (no hats or sunglasses; No polaroids)
4. Open Registration Form AND a. Fill out and Print out a registration form per player. b. Sign and date on the the appropriate area of form. c. Submit the orignal signed form and a copy of the form on one of the designated early registration dates, or if regular registration, to the soccer office on designated dates/times.
